Important Things to Know for 2012
- The Farmersville ISD Cafeteria Plan year runs from September 1, 2012 through August 31, 2013.
- The NEW Third Party Administrator for the Medical and Dependent Care Reimbursement accounts will be National Benefit Services, (NBS) for the plan year beginning September 1 through August 31. Claim forms can be found by clicking on the forms tab.
- The Medical and Vision premiums will increase effective September 1, 2012. To view the current plan summary and rates, click on Benefit and Forms to see the new plan information.
- LOYAL AMERICAN Cancer insurance continues as the cancer provider effective 9/1/12. This supplemental insurance pays in addition to any other coverage you may have. A plan summary and rates are available to view by clicking on the Benefits and Forms link.
- 5 STAR LIFE will be the group term life and AD&D carrier again effective 9/1/12. 5 Star allows you to enroll up to 7 times annual salary up to $150,000 in coverage for employee, 50% of employee election, up to $75,000 for spouse and 50% of employee election, up to $10,000 for dependent children. A Health application is required for anyone who wants to increase coverage. New Hires are guaranteed issue if application is done within 31 days of date of hire. - UNUM continues as the Disability carrier effective September 1. The rates and plan summary can be found under the Benefits and Forms link.
- SUPERIOR VISION continues as the vision carrier for the coming year. A plan summary and rates are available to view by clicking on the Benefit Highlights tab/ Vision.
- TEXAS PERMANENT Life insurance is also available to Farmersville ISD employees. This insurance offers term life coverage to age 120 and coverage can be purchased for dependents without having to purchase any for the employee. A plan summary and rates are available to view by clicking on the Benefit Highlights tab/ Permanent Life. Anyone who is interested in this insurance must contact FBS at 469-385-4640 to complete an application.
